Coastal Carolina Family Practice FollowMyHealth is a secure, convenient way to manage your personal health care (and your dependents) and communicate directly with your doctor’s office online. This FREE self-service health management tool can improve interactions with your doctor’s office, document important health care information and ultimately give you the ability to make more informed decisions about your health!

How do I sign up for Premier Medical Associates FollowMyHealth?

1. Visit any Premier Medical Associates office and present your photo ID to the reception staff, who will verify your identity and notify the Premier Medical Associates FollowMyHealth team that you’ve signed up.

2. Within 5 business days you will receive an email invitation from ‘noreply@followmyhealth.com’ to activate your Premier Medical Associates FollowMyHealth account. (Check your SPAM filter if you don’t see it in your inbox.)

3. Your email invitation will include a link to the Premier Medical Associates FollowMyHealth login page. Select the green "FMH Secure Login" icon and create a username and password for your account.

Optional: You can also login using an existing Windows Live ID, Google, Facebook or Yahoo account.

4. You will be guided through simple steps to complete the registration. First, the welcome screen initiates the process. Next you will enter an Invitation Code, which is the last four digits of your social security number or your year of birth. Finally, review the Terms of Use and Release of Information screens and click “I accept.”

5. Step 5 is your Premier Medical Associates FollowMyHealth record being populated with your information.
